Medical coding is the process of extracting data from a doctor's notes on a patient's medical history, including medical procedures, diagnoses, necessary medical equipment, and medical services. This data is converted into standard medical alphanumeric codes by a medical coder.
Medical Coders uses a set of codes like ICD, CPT and HCPCS codes for coding purposes. ICD codes are diagnostic codes which denotes diagnosis of a patient and contains, signs and symptoms, diagnosis, reason for illness and cure. These codes are published by WHO. For instance ICD code, F45.21 denotes Hypochondriasis.
